Form for Non-U.S. Citizens visiting General Atomics

For those persons visiting General Atomics for the ITPA meetings and who are not U.S. citizens, the following form has to be filled out.

General Atomics is obligated to comply with the U. S. Department of Energy policy 142.1 for persons visiting the site who are not citizens of the United States of America. All such persons must have approval prior to visiting the Fusion Group at General Atomics. If approval is not received, the non-U.S. citizen could be subject to not being allowed access to the GA site. The form below is only for persons who are not U. S. citizens. If you are a U. S. citizen you do not need to complete this form.

Note that this form can be submitted after May 16, 2008, but then there will be no gaurantee that the approval process will be completed prior to the meeting dates. So submit this form by May 16 even if you do not completely know that you will be attending the meetings.
